CHICAGO (WLS) — A South suburban woman who has been accused of using a fake vaccine card in Hawaii is wanted by police after pulling out of a virtual court hearing this week.

Oak Lawn’s Chloe Morozak allegedly used a fake COVID-19 vaccination card that had a misspelling of “modern” to go on vacation in Hawaii last week without needing 10 days.

Mirojak faces misdemeanor charges of violating the state’s coronavirus emergency proclamation, which requires visitors to show proof of full vaccination or a negative test to move freely across the islands.

According to a report filed by investigators at the Hawaii Attorney General’s office, screeners became suspicious of the 24-year-old Oak Lawn resident’s alleged CDC card when he checked Daniel K. found the arrival.

The card claims that Marozak received his first “Madrana” vaccine dose from a National Guard member named “CPL Wolf” in Delaware, with the second dose “SSgt Monte”, photocopies of the purported document show.

Officials said Mirojak still made it through screening before investigators determined the hotel and that the return flight information listed in his state-mandated travel record was bogus. Public health officials in Delaware later told investigators they had no records of shots fired in their jurisdiction.

Mirozak reportedly ignored investigators’ calls and emails over the next few days, so he waited for her at the airport for her return flight on Saturday.

When they arrested her at a Southwest Airlines counter, she reportedly made “a spontaneous utterance” that she would shoot him in a doctor’s office and pay for it, the report says.

He was charged with two counts of falsifying vaccination documents, a fine of up to $5,000, or a year in prison.

Now there is a warrant for his arrest.
